Utility cover in road crossing, in pedestrian sight line, has become a pot hole causing dangerous trip hazzard

This pot hole has been here for 9 months, this is extremely dangerous as it sits exactly in the pedestrian crossing sight line at a controlled crossing in front of the large Sainsbury's and M&S where there is a high volume of pedestrian and cyclist crossing day and night. to access the shops and Wandle river path.
