3 inch hole in cover on pavement

This is on a school route. My daughters scooter wheel went into this this morning, flipped her scooter and she went flying. Grazes both her palms and a huge graze on her elbow. This needs sorting ASAP as I mentioned, it's one of the main routes to Garfield Primary School.
