Sent to Merton Council less than a minute later. Council ref: SRQ-1259753-C5M3C1.

A paving slab is raised the slab is between Hebert Road and Hartfield Road Looking from Herbert Road towards Hartfeild on Left side. There is a tree which is the problem. I was thrown off my mobility scooter by this slab.
